We are aware that some patients might be understandably anxious about the Government's recent announcements around the use of face masks in various public settings. GPs are unfortunately not in a position to provide individual risk assessments or letters for patients who feel that they should be exempt from wearing a face mask. The government guidance on exemptions suggests there is no requirement for evidence for exemption therefore it is sufficient for an individual to self-declare this.

REMINDER: Would all patients attending for Asthma appointments please remember to always bring along their inhalers. This clinic provides a service for patients who currently have heart disease. It offers advice about maintaining health and reducing the possibility of any further heart problems. It is run by specially trained Nurses and assessed by invitation only.

New patients must live within these boundaries to be able to register with us. You can register by bringing along your NHS medical card and completing a GMS1 registration form and a new patient questionnaire - these are available to download and print in our patient download library.
