Carers Information
read moreDo you regularly, sometimes all round the clock, without payment, look after someone else who has additional needs or health problems, maybe a partner, parent, child, friend or neighbour? If the answer is YES, please ensure that you let the surgery know. You will then be registered here as a carer which means that your caring commitments can be taken into account and the practice will do its best to accommodate you with suitably timed appointments whenever possible.
New Patients
read morePlease telephone 029 2076 4444, or visit reception for details. The forms are available to download below (along with some guidelines for completing the GMS1W form). If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient. You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP.

Information for Patients
read moreThis page contains information to support various services provided by the surgery and the surrounding area. Losing a loved one can be a very difficult time. We aim to support you through this process as much as we can here at North Cardiff Medical Centre. It can take some time to process and complete a death certificate following a death. A death certificate is a legal document and can only be completed by a doctor who has met the deceased or in discussion with a coroner. Not all doctors work full-time and sometimes the necessary doctor is not immediately available.

Patient Record
read moreIncreasingly, patient medical data is shared e.g. between GP surgeries and District Nursing, in order to give clinicians access to the most up to date information when attending patients. The systems we operate require that any sharing of medical information is consented to by patients beforehand. Patients must consent to sharing of the data held by a health provider out to other health providers and must also consent to which of the other providers can access their data. In this case, patients would allow the surgery to share their data, they would allow the district nurses to access it but they would not allow access by the podiatry department.
