Anyone who has not received their appointment letter yet and fall into one of the eligible groups, should await receipt of the letter rather than contacting the team at this stage.

This service is only available for patients over 16 years old. 2 hand the form to our reception team who will process the details, before a letter is sent to your home with your registration details. Once you have received your Registration Letter you are ready to access the online services.
